Eligibility Requirements and Application for Individuals Seeking Assistance
The SA Supportive Housing Program is intended to address the needs of individuals stepping down from hospital and non-hospital drug and alcohol rehab or halfway house programs. To be approved, the applicant seeking assistance through this program must:
- Require financial assistance to move into a Recovery House as part of his/her plan of recovery from substance abuse. This may be demonstrated in one of two ways:
- A substance abuse professional associated with a hospital or non-hospital rehabilitation facility or halfway house must certify that the applicant requires assistance to "step down" from inpatient rehabilitation to a Recovery House as part of his/her recovery plan.
or
- A substance abuse professional associated with the Single County Authority in the applicant's county of residence must certify that the applicant requires assistance to enter a Recovery House as part of his/her recovery plan.
- Be or have been a resident of Cumberland, Dauphin, Lancaster, Lebanon, or Perry County for a period of no less than two (2) consecutive months prior to the date of application to the SA Supportive Housing Project.
Note that an individual is considered a Cumberland, Dauphin, Lancaster, Lebanon or Perry County resident when he/she has lived in Cumberland, Dauphin, Lancaster, Lebanon or Perry County for a period of not less than two (2) consecutive months prior to the date service is requested, excluding jail time, hospitalization, and institutionalization.
